The below equipment waiver is for club student-athletes playing in the collegiate club ranks. Please be sure to include the student-athlete’s name and cap number, an explanation of the equipment and a photo of the equipment when submitting your request electronically. Forms can be submitted to either commissioner@collegiatewaterpolo.org or tracey@collegiatewaterpolo.org for approval.

By rule, the NCAA requires varsity student-athletes to receive permission for the use of protective equipment directly from the NCAA Secretary of Rules Editor for the sport of Water Polo. Teams must receive written confirmation of approval that should be made available to the officials prior to the start of every game. The request should be submitted by the varsity coach or varsity school trainer and can be an emailed to Brian Streeter, NCAA Secretary of Rules Editor (bfs6@psu.edu).
